PDE4 inhibitors for the treatment of atopic dermatitis

PDE4 inhibitors have been the subject of intense pharmaceutical investigation for a number of years now. This therapeutic class is of particular use for the treatment of a variety of inflammatory diseases. For example PDE4 inhibitors were highlighted in our recent dossier "Therapeutic and pharmaceutical approaches to COPD" since the PDE4 inhibitor, cilomilast has some beneficial clinical effect in patients with either emphysema or bronchitis. This pharmaceutical class may also be of benefit to patients with atopic dermatitis. This pruritic inflammatory skin disease affects 10% of the population at some time in their life with about 60% of them continuing to have symptoms into adulthood. More than 15 million people have symptoms of the disease in the US alone. This condition often begins in infancy and frequently affects individuals with personal or family history of atopic disease. The world market for drugs to treat allergic diseases is a massive £10 billion and is growing at over 10% per annum. New products in current development offer little therapeutic advantage or improved patient compliance over existing therapy. German-based researchers have recently shown that two PDE4 inhibitors, cilomilast and AWD 12-281 are therapeutically active in a model of allergic dermatitis in mice. This study therefore offers proof supporting the concept that PDE4 inhibitors may be of use in the treatment of this disease.
