QTest Labs provides economical and rapid testing between drug discovery and preclinical trials . With a 2 week turn-around time and before conventional elaborate and expensive preclinical trials. QTest evaluates compounds for their effects on heart rate (RR interval), ventricular repolarization (QT and QTc), conductivity (P, PQ and QRS durations/intervals), inotropy (dP/dtmax), lusitropy (dP/dtmin) and coronary vascular resistance (dynes s cm-5). Studies are conducted on isolated perfused guinea pig hearts (Langendorff preparations) contracting isovolumetrically on a fluid-filled balloon. Recordings of high quality are of a bipolar cardiac electrogram, left ventricular pressure (LVP), and dLVP/dt. Coronary blood flow at a constant perfusion pressure is collected for 15 seconds. Most drug regulatory agencies accept studies conducted on isolated, perfused guinea pig hearts as a reasonable part of a comprehensive plan for drug safety evaluation. The value of using the Langendorff preparation is that it contains all tissues (e.g. Purkinje fibers, normal myocardium, “M” fibers) whose ionic channels may be affected by drugs. Therefore, although this preparation may be less sensitive than HERG channels or isolated superfused Purkinje fibers if a compound exerts toxicity on those structures, if the compound exerts potential toxicity by affecting some structures other than HERG channels or Purkinje fibers there is a possibility of identifying that potential toxicity in the Langendorff preparation possessing all cardiac tissues.
