VillageMD, a national primary care provider, has expanded into Northern Indiana to help primary care physicians across the region deliver higher value care. VillageMD-Northern Indiana will provide solutions and services to its provider partners to improve clinical outcomes, enhance patient and physician satisfaction, and lower the total cost of care.

“High-quality primary care requires a deep engagement between doctor and patient, and VillageMD’s proven clinical model provides the analytical insights, personalized services and integrated care teams to best nurture this relationship across the healthcare system,” said Chiedu Nchekwube, M.D., Family Medical and Wellness Center, and a member of the VillageMD-Northern Indiana network. “VillageMD came to our rescue by providing the resources needed to restore the joy of patient care and an economic model that rewards us for delivering better care at an efficient total cost.”

VillageMD-Northern Indiana will leverage VillageMD’s data analytics, physician-based care coordination, and on-the-ground support resources, all of which have demonstrated significantly greater clinical improvements at a lower total cost of care. VillageMD’s clinical care model also aids its physician partners in providing more personalized attention, education, and support to patients via integrated care teams of health coaches, diabetes educators, pharmacists, and resource coordinators to address patients’ medical, emotional and social needs.

“We are excited to bring our enhanced model of primary care to physicians in Northern Indiana, especially at a time when they are seeking greater resources, support and validation,” said Tim Barry, chief executive officer, VillageMD. “Together, we can transform the delivery of primary care and help these practices not just compete but grow in the era of value-based reimbursement.”

VillageMD’s high-value model of care is proven to drive results across multiple markets and across all patient populations – commercial, Medicare, Medicare Advantage and Medicaid. Practices that have adopted VillageMD’s patient-centric approach have been recognized by the Centers for Medicare and Medicaid Services for high quality and high-performance metrics, such as admissions and readmissions rates 20 to 45 percent lower than the community, Medicare costs 20 to 45 percent lower than the community, and commercial costs 15 to 20 percent lower than the community.

VillageMD’s model has been shown to improve clinical outcomes and lower the total cost of care in chronic disease populations like diabetes where patients have a 93 percent medication adherence rate; hemoglobin A1c control is 63 percent higher, potentially preventable admissions rates are 36 percent lower, and the total cost of their care tracks more than 20 percent lower as compared to community standards.
