Manipulation under Anesthesia: Does Polyethylene Thickness Matter?

Total knee arthroplasty (TKA) has been established as the most effective treatment for end-stage, symptomatic osteoarthritis of the knee. However, improper polyethylene size selection has been proposed to predispose patients to postoperative stiffness following TKA. The aim of this study is to evaluate if there is a correlation between the use of the thinnest tibial implant thickness and implant manufacturer with the likelihood of undergoing manipulation under anesthesia (MUA). A retrospective review of uni...

Palladium-Catalyzed Double Carbonylative Cyclization of Benzoins: Synthesis and Photoluminescence of Bis-Ester-Bridged Stilbenes.

A palladium-catalyzed double carbonylative cyclization of benzoins has been developed, which realizes the synthesis of bis-ester-bridged stilbenes just in two steps from aldehydes. Thus, the obtained fully fused tetracyclic π-systems have a pyrano[3,2- b]pyran-2,6-dione (PPD) core on their center, showing two reversible reductions at low potentials. In addition, their photoluminescence properties are strikingly affected by the aromatic rings fused to the PPD core; bis- thieno-fused PPDs are found to be exc...

Male-to-Female Gender Confirmation Surgery: Intestinal Vaginoplasty.

The major steps in vaginoplasty are orchiectomy, penile amputation, creation of the neovaginal cavity with lining, and reconstruction of urethral meatus, labia, and clitoris. During pedicled intestinal transfer, an intestinal segment is transferred in a dissected cavity between the bladder and rectum. The bowel harvest is performed by a total laparoscopic technique. It is imperative to create the labia majora and minora, the clitoris, and a clitoral hood to achieve the physiologic and aesthetic equivalent o...

QM/MM study of the reaction mechanism of Cl-cis,cis-muconate with muconate lactonizing enzyme.

The lactonization process of Cl-cis,cis-muconate catalyzed by anti-muconate lactonizing enzyme (anti-MLE) was studied theoretically with the aid of a combined quantum mechanics/molecular mechanics (QM/MM) approach. Two elementary processes steps involved in the lactanization process were investigated. The calculated energy barriers agree well with the experimental values. The present work provided the explicit structures of the enolate anion intermediates. The electrostatic influence analysis highlighted re...

The Clinicopathological and Prognostic Significance of IDO1 Expression in Human Solid Tumors: Evidence from a Systematic Review and Meta-Analysis.

Indoleamine 2,3-dioxygenase 1 (IDO1) is a heme-containing enzyme catalyzing the initial and rate-limiting steps in the kynurenine pathway, which converts tryptophan into kynurenine. Upregulation of IDO1 decreases tryptophan levels and increases the accumulation of kynurenine and its metabolites. These metabolites can affect the proliferation of T cells. Increasing evidence has shown that IDO1 is highly expressed in various cancer types and associated with poor prognosis of cancer patients. However, the resu...

Interesting case of late Gore-Tex extrusion following medialization laryngoplasty.

A 65-year-old female presented with a foreign body sensation following an asthmatic attack associated with severe coughing. Six years earlier, the patient underwent medialization laryngoplasty (ML), which was complicated by a small tear (2 mm) in the right ventricle. One year following this, the patient developed Gore-Tex extrusion but elected only for partial removal. Healing was complete until 5 years later; on examination, the patient had evidence of Gore-Tex extrusion through the right ventricle. Implan...

ZINClick v.18: Expanding Chemical Space of 1,2,3-Triazoles.

In the last years, we have investigated the click-chemical space covered by molecules containing the triazole ring, we generated a database of 1,2,3-triazoles called ZINClick, starting from literature-reported alkynes and azides synthesizable in no more than three synthetic steps from commercially available products. This combinatorial database contains millions of 1,4-disubstituted-1,2,3-triazoles that are easily synthesizable. The library is regularly updated and it is freely downloadable (http://www.ZINC...

Organo-Cation Catalyzed Asymmetric Homo/Hetero-Dialkylation of Bisoxindoles: Construction of Vicinal All-Carbon Quaternary Stereocenters and Total Synthesis of (-)-Chimonanthidine.

A novel chiral spirocyclic amide (SPA)-derived triazolium organo-catalyst has been designed and demonstrated to effect asymmetric homo and hetero dialkylations of various bisoxindoles, enabling enantioselective construction of vicinal all-carbon quaternary stere-ocenters. These reactions feature excellent enantio- and diastereo-selectivities (up to 99% ee and >20:1 dr), as well as good to high yields (up to 89% over two steps). As an application of this meth-odology, the first asymmetric total synthesis of ...

Sample preparation in foodomic analyses.

Representative sampling and adequate sample preparation are key factors for successful performance of further steps in foodomic analyses, as well as for correct data interpretation. Incorrect sampling and improper sample preparation can be sources of severe bias in foodomic analyses. It is well known that both wrong sampling and sample treatment cannot be corrected anymore. These, in the past frequently neglected facts, are now taken into consideration, and the progress in sampling and sample preparation in...

Comparative Studies of the Biosynthetic Gene Clusters for Anthraquinone-Fused Enediynes Shedding Light into the Tailoring Steps of Tiancimycin Biosynthesis.

Comparative analyses of the four known anthraquinone-fused enediynes biosynthetic gene clusters identified four genes, tnmE6, tnmH, tnmL, and tnmQ, unique to the tnm gene cluster. Larger scale fermentation of both the S. sp. CB03234 wild-type and the Δ tnmH and Δ tnmL mutant strains resulted in the characterization of 20 new tiancimycin (TNM) congeners, including five enediynes. These findings enabled a proposal for the late stage of TNM biosynthesis featuring an intermediate possibly common for all anthr...

Visible Light-induced Radical Cyclization of Tertiary Bromides with Isonitriles to Construct Trifluoromethylated Quaternary Carbon Center.

The reaction of trifluoromethylated tertiary bromides with isonitriles induced by visible light is reported. Defluorination could be avoided in a radical process. This method provides an efficient approach to compounds containing a trifluoromethylated quaternary carbon center, most of which show excellent potential to be agrochemicals. In addition, the bromides were pre-pared from perfluoroisobutylene after several steps, which is a waste from industry. This reaction shows a feasible transfer of harmful was...

A synthetic study toward the core structure of (-)-apicularen A.

A concise synthetic strategy towards the core structure of (-)-apicularen A has been described in an 11-step synthetic sequence from a known intermediate. The key steps include tandem isomerization followed by C-O and C-C bond-forming reactions and iodocyclization strategies for the synthesis of a bicyclic ether embedded in the macrolactone ring. The applied reagent-controlled Keck-Maruoka allylation, Lin Pu alkynylation and Ricket-Diels-Alder reactions were used to simplify the synthetic sequence of relate...

Ensifer-Mediated Transformation (EMT) of Rice (Monocot) and Oilseed Rape (Dicot).

Ensifer adhaerens OV14 underpins the successful crop transformation protocol, termed Ensifer-mediated transformation (EMT). The adaptability and efficiency of EMT technology to successfully transform both monocot and dicots have been previously reported. To facilitate community users' transition to EMT, the modified rice and oilseed rape plants generated in this work were developed using EMT protocols that were grounded in standard Agrobacterium-mediated transformation (AMT) processes. Therefore, this chapt...

Expression of IgG Monoclonals with Engineered Immune Effector Functions.

The therapeutic development of monoclonal antibodies requires robust and reliable methods for their recombinant expression and characterization. In this context, an increasingly important aspect in the antibody development process is to determine the contribution of Fc-mediated immune effector functions to therapeutic activity. Here we describe steps for the cloning and mammalian expression of mouse and human IgG monoclonals with reduced immune effector functions, based on mutation of Fc-gamma receptor and ...

Arthroscopic Treatment of Ankle Instability - Allograft/Autograft Reconstruction.

Inversion ankle sprains represent one of the most common traumatic injuries in the active sports population. Although most respond well to conservative treatment, some hide important lesions. Lateral ankle ligament injuries occur in more than 80% of all ankle sprains, with one-third of these developing chronic ankle instability (CAI). Lateral ankle ligament repair or reconstruction procedures aim to restore normal ankle anatomy and function in patients with CAI. Arthroscopic reconstruction techniques allow ...

EPR Lineshape Analysis to Investigate the SNARE Folding Intermediates.

SNARE complex formation, which is believed to drive intracellular membrane fusion, transits through multiple conformational states along the membrane fusion pathway. The SNARE intermediates are biologically important because they serve as targets for fusion regulators and clostridial neurotoxins. Spin-labeling EPR has contributed significantly to the understanding of the structures and the dynamics of SNARE intermediates. In particular, the EPR lineshape analysis, which is highly sensitive to protein confor...

Identification of adeno-associated virus capsid proteins using ZipChip CE/MS.

A simple and rapid identity test of adeno-associated virus (AAV) serotypes is important for supporting the AAV gene therapy development, as it relates to its efficacy and safety. The current mass spectrometry-based identity tests require extensive sample preparation steps, relatively large sample quantities and long analysis time. Herein, we describe a simple and novel microfluidic ZipChip CE/MS method used to characterize AAV capsid proteins. The three capsid proteins of AAV2 were separated and identified ...

Spectrally resolved thermal emission of atmospheric gases measured by laser heterodyne spectrometry.

The demonstration of thermal infrared quantum cascade laser heterodyne spectrometry to resolve local thermodynamic equilibrium molecular emission lines from earth's atmospheric constituents is presented. The instrument is described, as well as the early steps towards radiometric calibration. Room temperature ethylene emission line measurements carried out in the laboratory are used to validate the instrument. High-resolution (0.02  cm) emission lines from atmospheric carbon dioxide and water vapor are t...

Perspectives on preoperative systemic treatment and breast conservative surgery: One step forward or two steps back?

Preoperative systemic therapy (PST) has become an accepted treatment not only for locally advanced but also for early stage breast cancer patients. Clinical trials have demonstrated that the use of PST is equally to adjuvant treatments in terms of overall survival, and has the advantage of increasing rates of breast conservative surgery and rates of pathologic complete response, a surrogate endpoint for the effectiveness of systemic therapy. Initial studies have suggested higher rates of locoregional recurr...

Transcriptome Profiling of Single Mouse Oocytes.

Single-cell RNA-sequencing (scRNAseq) enables the detection and quantification of mature RNAs in an individual cell. Assessing single cell transcriptomes can circumvent the limited amount of starting material obtained in oocytes or embryos, in particular when working with mutant mice. Here we outline our scRNAseq protocol to study mouse oocyte transcriptomes, derived from Tang et al., Nat Methods 6(5):377-382, 2009 . The method describes the different steps from single cell isolation and cDNA amplification ...

Extracting DNA from FFPE Tissue Biospecimens Using User-Friendly Automated Technology: Is There an Impact on Yield or Quality?

DNA extracted from formalin-fixed, paraffin-embedded (FFPE) tissue blocks is amenable to analytical techniques, including sequencing. DNA extraction protocols are typically long and complex, often involving an overnight proteinase K digest. Automated platforms that shorten and simplify the process are therefore an attractive proposition for users wanting a faster turn-around or to process large numbers of biospecimens. It is, however, unclear whether automated extraction systems return poorer DNA yields or ...

Roles of ubiquitin in autophagy and cell death.

The balance between cell survival and cell death is often lost in human pathologies such as inflammation and cancer. Autophagy plays a critical role in cell survival: essential nutrients are generated by autophagy-dependent degradation and recycling of cellular garbage. On the other hand, cell death is induced by different programs, such as apoptosis, pyroptosis, and necroptosis. Emerging evidence is revealing how cell survival and cell death pathways are coordinated to determine cell fate. For instance, po...

Factors Favoring and Impairing Healing of Tracheal Anastomosis.

The factors governing successful healing of and impairing of tracheal and bronchial anastomosis are best understood by reviewing the normal histologic changes accompanying healing, governing factors, and biochemical advances made in the last 5 decades. Normal wound healing factors, also relevant to tracheal and bronchial reconstruction, rely on precise handling of tissues without interfering with tissue perfusion, careful selection and placement of sutures, and steps to minimize tension. Impairments of sati...

Unwanted Side Effects in Children and Youth Psychotherapy - Introduction and Recommendations.

Psychotherapy in children and adolescents is effective, but unwanted effects can occur. Until now, psychotherapy research has neglected this important topic, although children and youths are in need of special protection. Unwanted effects caused by therapy are not systematically investigated and a corresponding conceptualization is missing. The aim of this article is to investigate whether the current classifications of unwanted effects of psychotherapy in adults are applicable to children and adolescents a...
