Comparison between single portal endoscopic, 1-cm open carpal tunnel release.

Given the controversy about the best surgical technique for carpal tunnel release, this study tested the hypothesis that no significant differences would be observed between single-portal endoscopic release and a short, 1-cm incision open release in a large sample of patients. Consecutive patients were assigned to one of the two techniques. Preoperative and postoperative measurements included grip and pinch strength, a visual analog scale for pain and a satisfaction questionnaire. Eighty-seven patients comp...

Fourth branchial pouch sinus: A report of 7 cases and review of the literature.

A fourth branchial pouch sinus often manifests quite late in life as a recurrent neck abscess, suppurative thyroiditis, or pseudothyroiditis. Demonstration of the sinus opening in the piriform fossa by hypopharyngoscopy in combination with ultrasonography of the neck provides adequate information to justify proceeding to surgery. The sinus tract usually courses through the thyroid cartilage. The most effective treatment is surgical excision of the tract up to the piriform fossa through the cartilage. This p...

Molecular characterization of Corynebacterium diphtheriae isolates in Malaysia between 1981 and 2016.

Sporadic diphtheria cases in Malaysia have remained low in number since the 1990s. However, in 2016 a total of 31 cases were reported nationwide and to investigate this we performed molecular characterization of 30 Corynebacterium diphtheriae isolates collected from 1981 to 2016 using multilocus sequence typing (MLST). C. diphtheriae isolates were identified and biotyped using the API Coryne kit, while the toxigenicity was determined by PCR and the Elek test. All of the 2016 isolates belonged to biotype mit...

Combining carbon pricing with LPG subsidy for promoting preservation and restoration of Uttarakhand forests.

This paper explores the role of REDD+ programs in promoting community led preservation and restoration of forests in the Himalayan region of India. As there often exists a direct conflict between generating carbon credits through forest conservation and forestry-based livelihood sustenance activities, we consider the role of cooking gas subsidy in reducing communities' reliance on forests. Findings indicate that a price of 13 USD per ton of carbon dioxide equivalent can induce a switch away from fuelwood to...

The Fourth Hurdle.

The fourth hurdle in drug development and patient access to new therapeutics is reimbursement. The evidentiary requirements that are sufficient to regulators may not be sufficient to allow health technology assessment (HTA) bodies to make informed cost-effectiveness and formulary decisions. The accompanying paper by Vreman et al confirms that facilitated regulatory pathways may result in regulatory approvals but may not result in patient access to new and important medicines. This article is protected by c...

Association between rapid utilisation of direct hepatitis C antivirals and decline in the prevalence of viremia among people who inject drugs in Australia.

The World Health Organization (WHO) established targets to eliminate hepatitis C virus (HCV) infection as a public health threat by 2030. Evidence that HCV treatment can lower viraemic prevalence among people who inject drugs (PWID) is limited. Broad accessibility of direct-acting antiviral (DAA) therapy in Australia, since March 2016, provides an opportunity to assess the efficacy of these treatments at a population level in a real-world setting.

Lower Quarter Y-Balance Test in healthy women 50-79 years old.

The Lower Quarter Y-Balance Test (LQ-YBT), a measure of dynamic balance used in young adults, has not been adequately studied in older women. We determined the reliability, normative values, and relationships to other balance measures for LQ-YBT in women aged 50-79 years. Interrater reliability was strong, and test-retest reliability was moderate to strong. Results by decade showed women 50-59 years had significantly better scores than both older groups. There were moderate positive correlations between s...

To understand the effects of macrophyte growth on water environment and rotifer community structure, monthly survey lasted for one year from October 2015 to September 2016 was conducted in Lake Minghu and Lake Nanhu at Jinan University in Guangzhou. There was no macrophyte cultivation in Lake Minghu. A kind of macrophyte Hydrilla verticillata grows from March-October in Lake Nanhu and grows vigorously in June-September. In this study, 50 species belonging to 23 gene-ra of rotifer, were recorded in the two l...

The prevalence and years lived with disability caused by low back pain in China, 1990 - 2016: findings from the global burden of disease study 2016.

The aim of this work was to quantify the prevalence and years lived with disability (YLDs) caused by low back pain (LBP) in China from 1990 to 2016. Data from the GBD 2016 (Global Burden of Diseases, Injuries, and Risk Factors Study 2016) were used. We analyzed the age-sex-province-specific prevalence and YLDs for LBP of 33 provinces/regions in China. Comparisons were made with the data retrieved from the 1990 GBD study. We estimated that 5.45x10 individuals had LBP in 1990, which rose to 6.73x10 in 2016. T...

All patients who started SV from October 2016 to March 2017 on an outpatient basis were included.A total of 427 patients started treatment with SV. Mean follow-up was 7.0±0.1 months. Forty-nine patients (11.5%) discontinued SV, and 12 (2.8%) died. SV discontinuation was associate...

Randomized Feasibility Trial of a Low Tidal Volume-Airway Pressure Release Ventilation Protocol Compared With Traditional Airway Pressure Release Ventilation and Volume Control Ventilation Protocols.

Low tidal volume (= tidal volume ≤ 6 mL/kg, predicted body weight) ventilation using volume control benefits patients with acute respiratory distress syndrome. Airway pressure release ventilation is an alternative to low tidal volume-volume control ventilation, but the release breaths generated are variable and can exceed tidal volume breaths of low tidal volume-volume control. We evaluate the application of a low tidal volume-compatible airway pressure release ventilation protocol that manages release ...

NMDA Receptors potentiate activity-dependent dendritic release of neuropeptides from hypothalamic neurons.

Using "sniffer" cell biosensors, we evaluated the effects of specific firing patterns and frequencies on activity-dependent somatodendritic release of VP from PVN neurones. Somatodendritic release of VP was rarely observed during continuous firing but was strengthened by clustered activity. Moreover, release evoked at any given frequency was robustly potentiated by NMDAR-mediated firing. Differently from axonal release, NMDAR activation was necessary for somatodendritic release to occur at physiological fir...

Unc13: a multifunctional synaptic marvel.

Nervous systems are built on synaptic connections, and our understanding of these complex compartments has deepened over the past quarter century as the diverse fields of genetics, molecular biology, physiology, and biochemistry each made significant in-roads into synaptic function. On the presynaptic side, an evolutionarily conserved core fusion apparatus constructed from a handful of proteins has emerged, with Unc13 serving as a hub that coordinates nearly every aspect of synaptic transmission. This revie...

Alberta wildfire 2016: Apt contribution from anomalous planetary wave dynamics.

In May-June 2016 the Canadian Province of Alberta suffered one of the most devastating wildfires in its history. Here we show that in mid-April to early May 2016 the large-scale circulation in the mid- and high troposphere of the middle and sub-polar latitudes of the northern hemisphere featured a persistent high-amplitude planetary wave structure dominated by the non-dimensional zonal wave number 4. The strongest anticyclonic wing of this structure was located over western Canada. In combination with a ver...

Understanding the developmental dynamics of M. hapla can improve the timing of diagnostic sampling and nematicide application. Three Vitis vinifera vineyards in Washington were sampled March 2015 to March 2017 to determine the developmental dynamics of M. hapla by measuring second-stage juveniles (J2) in soil, eggs and adult females in roots, and fine root tips. A model of M. hapla J2 development based...

A Self-Actuated Cellular Protein Delivery Machine.

Engineered bacterial cells have great promises to solve global problems yet they are hampered by lack of convenient strategy for controlled protein release. A well-controlled protein translocation through cellular membranes is essential for cell-based protein delivery. Here we have developed a controlled protein release system by programming a bacterial autotransporter system named Ag43. Ag43 protein is engineered by adding a protease digestion site between its translocation and cargo domains. Once it is di...
