Pain in people with cancer is common but often under-recognized and under-treated. Guidelines can improve the quality of pain care, but need targeted strategies to support implementation.
Airway management is a major competence of clinical anesthesia. Knowledge of available materials and skills in the use of specific methods and techniques are essential and should be trained routinely. In 2011 the scientific working group in pediatric anesthesia (WAKKA) of the German Society for anesthesia and intensive care medicine (DGAI) published a treatment recommendation for the management of the unexpected difficult pediatric airway. Furthermore, a recommendation for the management of the expected dif...
Fasciolosis is a worldwide disease caused by the liver fluke Fasciola spp. This food- and water-borne disease is a major public health and veterinary issue. It is currently (re)emerging in several regions mainly due to the rapid evolution of human activities. This article reviews the current knowledge of the impact of irrigation-system management, livestock management, and human diet and hygiene habits on the emergence of fasciolosis. We also identify the gaps in this knowledge and the possible solutions fo...
Cervical cancer is common in resource-poor settings with high prevalence of tuberculosis, pelvic inflammatory disease, and human immunodeficiency virus (HIV) infection. There are no data regarding the sentinel lymph node (SLN) algorithm in these high-risk cancer populations. Our objectives were to establish the sensitivity, specificity, positive predictive value, and negative predictive value of the SLN algorithm in cervical cancer and to compare the detection rate of indocyanine green (ICG) versus blue dye...
Management options for cystic echinococcosis (CE) remain a serious problem. The main aim of this study was to examine the selection and complications of treatment applied in patients with CE. The second aim was to evaluate the mortality rate and causative factors.
Plants present a variety of defensive strategies against herbivores, broadly classified into tolerance and resistance. Since resource availability can also limit plant growth, we expect plant allocation to resource acquisition and defense to vary along resource gradients. Yet, the conditions under which one defensive strategy is favored over the other are unclear. Here, we use an eco-evolutionary model to investigate plant adaptive allocation to resource acquisition, tolerance, and resistance along a resour...
In the United States, chronic pain is experienced by over 39.4 million adults, many of whom are treated with opioid pain medications.
The problem of stigma in mental health in connected to the problem of human rights. Mental health professionals, psychiatrists included, display stigmatization of people with mental disorders mostly through social distance, even though they have "politically correct" opinions about their patients. Negative beliefs and attitudes in psychiatric profession should be opposed because of their important influence on public opinion, opinion of patients' themselves and of their family members. The "disabling enviro...
This paper investigates the Locally Managed Marine Area (LMMA) approach through looking at developments and challenges of community-based marine resource management over time, with a particular focus on Fiji in the South Pacific region. A diachronic perspective, based on two multi-method empirical studies, is used to exemplify the social complexities of the implementation of this LMMA approach in a specific island setting. This perspective connects local stakeholders' establishment and management of a LMMA ...
To assess the methodologic quality of guidelines for the management of low back pain (LBP) and compare their recommendations.
Testing for high-risk human papilloma virus (HR-HPV) is an effective approach to the prevention of cervical cancer. This study in the Atsinanana area of Madagascar aimed to compare the management of women screened by visual inspection after coloration with acetic acid (VIA) and the management of women screened by HPV with VIA as a triage test.
Marine policy and management has to cope with a plethora of human activities that cause pressures leading to changes to the natural and human systems. Accordingly, it requires many policy and management responses to address traditional, cultural, social, ecological, technical, and economic policy objectives. Because of this, we advocate that a fully-structured approach using the IEC/ISO 31010 Bow-tie analysis will allow all elements to be integrated for a cost-effective system. This industry-standard system...
There remains significant controversy behind the Surviving Sepsis Campaign Guidelines, as evidenced by the Infectious Diseases Society of America refusing to endorse the latest iteration put forth by the Society of Critical Care Medicine and the European Society of Intensive Care Medicine. An important stakeholder in this debate has not yet been adequately represented: hospitalists. Areas covered: In this perspective piece we review the key points in this controversy, specifically the current guidelines for...
Neurocognitive impairment remains a common complication of HIV despite effective antiretroviral therapy (ART). We previously reported improved neurocognitive functioning with ART initiation in seven resource limited settings (RLS) countries for HIV+ participants from AIDS Clinical Trials Group (ACTG) 5199 (International Neurological Study (INS)). Here we apply normative data from the International Neurocognitive Normative Study (INNS) to INS, to provide previously unknown rates of neurocognitive impairment.
Ineffective diabetes management results in suboptimal glycaemic control and adverse health outcomes. In resource-poor settings, a combination of high burden of medication nonadherence in patients and therapeutic inertia amongst clinicians is largely attributed to the failure to achieve glycaemic targets in diabetic populations. The potential health risks from intensification of medical therapy for aggressive lowering of glucose levels in Type 2 diabetes patients represents an ethical dilemma between avertin...
Human echinococcosis is a neglected infectious disease affecting more than one million people globally. Its diagnosis is expensive and difficult because of lack of adequate resources in low-resource locations, where most cases occur.
This paper spotlights human capital management, digital technology, and costs control as issues that healthcare leaders will face in redesigning the health care ecosystem in the 21st century.
The global zoo and aquarium community widely recognizes that its animal collections and cooperative breeding programs are facing a sustainability crisis. It has become commonly accepted that numerous priority species cannot be maintained unless new management strategies are adopted. While molecular data have the potential to greatly improve management across a range of scenarios, they have been generally underutilized by the zoo and aquarium community. This failure to effectively apply molecular data to col...
This practice resource is designed primarily as an educational resource for medical geneticists and other clinicians to help them provide quality medical services. Adherence to this practice resource is completely voluntary and does not necessarily assure a successful medical outcome. This practice resource should not be considered inclusive of all proper procedures and tests or exclusive of other procedures and tests that are reasonably directed to obtaining the same results. In determining the propriety o...
The majority of the five million perinatal deaths worldwide take place in low-resource settings. In contrast to high-resource settings, almost 50% of stillbirths occur intrapartum. The aim of this study was to synthesise available evidence of strategies for foetal surveillance in low-resource settings and associated neonatal and maternal outcomes, including barriers to their implementation.
The role of enriched autologous plasmas in androgenetic alopecia (AGA) management is emerging in recent literature.
Data about the optimal management of immunosuppressive therapy in liver transplant (LT) recipients with bloodstream infection (BSI) are missing. We aimed to describe the management of immunosuppressive therapy at diagnosis of BSI in LT recipients and to assess its impact on 28-day mortality.
To describe the use of cryopreserved ultra-thick human amniotic membrane for management of anophthalmic socket contracture.
The new guidelines for managing incidental pulmonary nodules published by the Fleischner Society in 2017 reflect an improved understanding of the risk factors and biologic features of lung cancer. Specific topics emphasized in the updated guidelines include a new threshold size for follow-up, the importance of the morphologic features of nodules, accurate nodule measurements, recognition of subsolid components, understanding interval growth or change in nodule morphology, and knowledge of patient risk facto...
