Stroke is the most feared complication of atrial fibrillation. To prevent stroke, left atrial appendage exclusion has been targeted, as it is the prevalent site for formation of heart thrombi during atrial fibrillation. We review the historic development of methods for exclusion of the left atrial appendage and the evidence to support its amputation during routine cardiac surgery.
The aim of the present report was to detail the advantages of using 3-dimensional (3D) prototyping in the planning, modeling, and manufacturing of an immediate palatal obturator for a 62-year-old man who underwent a left total maxillectomy to remove a solid, multicystic ameloblastoma. The prosthesis provided favorable restoration of stomatognathic functions, including speech, swallowing, and mastication. The use of an immediate obturator prosthesis made with 3D technology is an important aid in the treatmen...
The O-linked-N-acetyl-d-glucosaminylation (O-GlcNAcylation) modulates numerous aspects of cellular processes. Akin to phosphorylation, O-GlcNAcylation is highly dynamic, reversible, and responds rapidly to extracellular demand. Despite the absolute necessity to determine post-translational sites to fully understand the role of O-GlcNAcylation, it remains a high challenge for the major reason that unmodified proteins are in excess comparing to the O-GlcNAcylated ones. Based on a click chemistry approach, O-G...
Shoulder arthroplasty is a valuable option for treating glenohumeral osteoarthritis. Revision surgery for a failed shoulder arthroplasty is associated with difficult procedures, complications, and worse outcomes. Compared with a total joint arthroplasty, a resurfacing prosthesis has the supposed advantages of limited perioperative complications and little bone loss during revision. The aim of this study was to describe patient-reported outcome measures of revision surgery from failed uncemented Global CAP (...
This study sought to evaluate the impact of patient-prosthesis mismatch (PPM) on the risk of perioperative, early-, mid- and long-term mortality rates after surgical aortic valve replacement.
Our aim was to investigate amputation-free survival in people at high risk for foot ulceration in diabetes ('high-risk foot'), and to compare different subcategories of high-risk foot.
Click chemistry refers to a group of reactions that are fast, simple to use, easy to purify, versatile, regiospecific, and give high product yields. Therefore, a novel, efficient magnetic nano-sorbent based on N-methyl-D-glucamine attached to magnetic nanoparticles was prepared using click coupling method. Its boron sorption capacity was compared with N-methyl-D-glucamine direct attached nano-sorbent. The characterization of the magnetic sorbents was investigated by several techniques such as X-ray diffract...
This study aimed to investigate the possible influence of the RAP on dental implant osseointegration. Orthognathic surgery was set as an intervention for RAP, and a multi-center cohort study of two groups was undertaken. Group O included patients with single implant placement at least 4 months after orthognathic surgery and functional loading for more than 1 year, while controls (Group C) were without any major surgery. Clinical and radiographic assessments of implants, including changes in marginal bone le...
The glenoid component in reverse total shoulder arthroplasty is recommended to be positioned inferiorly or with a downward tilt with the intention of reducing scapular notching. However, it is still unclear whether modifying the position of the glenoid prosthesis affects implant stability. The aim of this study was to determine the association between implant positioning and glenoid prosthesis fixation using Grammont reverse total shoulder arthroplasty.
Cancer biomarkers are expected to be indicative of the occurrence of certain cancer diseases before the tumors form and metastasize. However, many biomarkers can only be acquired in extremely low concentrations, which are often beyond the limit of detection (LOD) of current instruments and technologies. A practical strategy for nanopore sensing of cancer biomarkers in raw human blood down to the femtomolar level is developed here. This strategy first converts the detection of cancer biomarkers to the quanti...
A 300-member flavonoid dimer library of multidrug resistance-associated protein 1 (MRP1; ABCC1) modulators was rapidly assembled using "click chemistry". Subsequent high-throughput screening has led to the discovery of highly potent (EC50 ranging from 53 to 298 nM) and safe (selective indexes ranging from >190 to >1887) MRP1 modulators. Some dimers have potency about 6.5- to 36-fold and 64- to 358-fold higher than the well-known MRP1 inhibitors, verapamil and MK571, respectively. They inhibited DOX efflux a...
Alternative, or ectopic, reservoir placement (ARP) has recently gained popularity among prosthetic surgeons in order to avoid injury to pelvic structures during inflatable penile prosthesis (IPP) implantation. However, the true incidence of complications associated with these new techniques remain understudied. We carried out a 5-year multi-center retrospective study with the aim to evaluate complications related to alternative reservoir placement.
Prosthesis-patient mismatch (PPM) after aortic valve replacement (AVR) may affect survival but data are conflicting. It is assessed by relating effective orifice area (EOA) to body surface area (EOAi). EOA is patient-specific as the result of flow-velocity times area at the individual patient's outflow tract levels (LVOT) divided by trans-prosthetic flow velocity. However, some studies use projected EOAs (i.e., valve size associated EOAs from other patient populations) to assess how PPM affects outcome.
Fast degradation of Mg-based implants results in the loss of mechanical integrity and poor osseointegration. Herein, a bilayer-structured coating (termed as HAT), comprising an outer layer of hydroxyapatite (HA) nanorods and an inner layer of pores-sealed MgO with HA/Mg(OH)2, was formed on Mg using plasma electrolytic oxidation and hydrothermal treatment. Osteoimmunomodulation, osseointegration, mechanical integrity, and bone-implant interfacial structure evolution of the HAT-coated Mg were investigated by ...
Structural valve deterioration represents the most important concern regarding the implantation of a biological heart valve in young patients. Several strategies have been proposed to improve prosthesis design in order to optimize performance and durability. Here we describe an early and specific mode for valve degeneration of prosthesis with outwards mounted pericardial leaflets due to pannus overgrowth on the aortic side. This led to leaflet retraction in two consecutive cases four and six years after the...
There is large variation in administration of performance-based, dynamic balance measures among adults with lower-limb amputation (LLA). Further, there has been limited exploration of test-retest reliability of these measures in adults with lower-limb loss, including whether there is a difference in reliability if one records 'best' versus 'average' performance across trials.
In this study, we present the working principle and conceptual design towards the realization of a fully-passive transfemoral prosthesis that mimics the energetics of the natural human gait. The fundamental property of the conceptual design consists of realizing an energetic coupling between the knee and ankle joints of the mechanism. Simulation results show that the power flow of the working principle is comparable to that in human gait and a considerable amount of energy is delivered to the ankle joint fo...
A majority of the nearly two million Americans living with limb loss suffer from chronic pain in the form of neuroma-related residual limb and phantom limb pain (PLP). Targeted muscle reinnervation (TMR) surgically transfers amputated nerves to nearby motor nerves for prevention of neuroma. The objective of this study was to determine whether TMR at the time of major limb amputation decreases the incidence and severity of PLP and residual limb pain.
Single radius knee implants were introduced to reduce the level of paradoxical anterior femoral translation (AFT) during mid-flexion after total knee arthroplasty. Findings from clinical and experiment studies are inconsistent, which may be due to the different loading conditions and articular conformities of the knee implants studied. The aim of this study is to analyze how variations in these two factors affect the mid-flexion stability of a single radius knee prosthesis.
We retrospectively evaluated a series of 75 surgical revisions after stapedotomy for the treatment of otosclerosis, carried out between 2001 and 2015. Intraoperative findings, causes of failure, and surgical solutions using an angular prosthesis, Causse prosthesis, and glass-ionomer cement were reviewed. Audiometric results performed the day before revision surgery and 1 to 2 months postoperatively were also examined. An incus necrosis was discovered in 65 patients; 55 of whom had partial necrosis of the lo...
Decellularized native extracellular matrix (ECM) biomaterials are widely used in tissue engineering and have reached clinical application as biomesh implants. To enhance their regenerative properties and postimplantation performance, ECM biomaterials could be functionalized via immobilization of bioactive molecules. To facilitate ECM functionalization, we developed a metabolic glycan labeling approach using physiologic pathways to covalently incorporate click-reactive azide ligands into the native ECM of a ...
In recent years, the use of surgically implanted biological aortic valves has been favored over mechanical prosthesis in patients between 50 and 70 years of age. However, outcomes on long-term survival are contradictory. The objective of this study was to determine if patients with mechanical valves have worse long-term survival than patients with biological prostheses.
This within-subject comparison tested the null hypothesis that there is no difference in patient satisfaction and oral health-related quality of life when an individual with an edentulous mandible is rehabilitated with a two-implant overdenture or a three-implant-supported fixed dental prosthesis.
Soft-tissue deficits in amputation stumps can lead to significant pain and disability. An emerging treatment option is stem cell-enriched fat grafting. This is the first study assessing the potential for this treatment modality in lower extremity amputation sites. In this prospective cohort study, five injured military personnel suffering from pain and limited function at amputation sites were recruited. Fat grafting enriched with stromal vascular fraction was performed at amputation sites to provide additi...
Popliteal arterial injury (PAI) is the second most common infrainguinal arterial injury after femoral artery injury with an incidence < 0.2%. A 2003 analysis of the National Trauma Data Bank (NTDB) reported a below the knee amputation (BKA) rate of 7.1% in patients with PAI as well as higher risk in those with an associated fracture or nerve injury. Given advances in vascular surgical techniques, improved multidisciplinary care and expeditious diagnosis with computed tomography angiography, we hypothesized ...
