Reducing overestimation of the treatment effect by interim analysis when designing clinical trials.

Several researchers in the statistical and medical communities have noted the overestimation of the treatment effect when a trial is stopped early in the interim analysis for efficacy; however, methods to reduce this overestimation are rarely used because the overestimation mechanisms are not well understood by many in clinical trial practice. A trial design that leads to less overestimation is needed.

Reporting interim results can show the feasibility of practicing outcome-based neurovascular care within randomized trials: an opinion.

Randomized trials of commonly performed surgical interventions are notoriously difficult to conduct. The trial methodology may nevertheless be the best way to offer outcome-based neurovascular care in the presence of uncertainty. One obstacle to promoting such trials that we will examine here is the conventional prohibition of publication and dissemination of interim results as the trial progresses.

Ectomycorrhizas and tree seedling establishment are strongly influenced by forest edge proximity but not soil inoculum.

Reforestation is challenging when timber harvested areas have been degraded, invaded by non-native species, or are of marginal suitability to begin with. Conifers form mutualistic partnerships with ectomycorrhizal fungi (EMF) to obtain greater access to soil resources, and these partnerships may be especially important in degraded areas. However, timber harvest can impact mycorrhizal fungi by removing or compacting topsoil, removing host plants, and warming and drying the soil. We used a field experiment to...

Landscape of Simple Superconformal Field Theories in 4D.

We explore the space of renormalization group flows that originate from supersymmetric N=1 SU(2) gauge theory with one adjoint and a pair of fundamental chiral multiplets. By considering all possible relevant deformations-including coupling to gauge-singlet chiral multiplets-we find 34 fixed points in this simple setup. We observe that theories in this class exhibit many novel phenomena: emergent symmetry, decoupling of operators, and narrow distribution of central charges a/c. This set of theories includes...

Lentiviral haemopoietic stem/progenitor cell gene therapy for treatment of Wiskott-Aldrich syndrome: interim results of a non-randomised, open-label, phase 1/2 clinical study.

Wiskott-Aldrich syndrome is a rare, life-threatening, X-linked primary immunodeficiency characterised by microthrombocytopenia, infections, eczema, autoimmunity, and malignant disease. Lentiviral vector-mediated haemopoietic stem/progenitor cell (HSPC) gene therapy is a potentially curative treatment that represents an alternative to allogeneic HSPC transplantation. Here, we report safety and efficacy data from an interim analysis of patients with severe Wiskott-Aldrich syndrome who received lentiviral vect...

Interim analysis incorporating short- and long-term binary endpoints.

Designs incorporating more than one endpoint have become popular in drug development. One of such designs allows for incorporation of short-term information in an interim analysis if the long-term primary endpoint has not been yet observed for some of the patients. At first we consider a two-stage design with binary endpoints allowing for futility stopping only based on conditional power under both fixed and observed effects. Design characteristics of three estimators: using primary long-term endpoint only,...

Interim PET-CT-guided therapy in elderly patients with Hodgkin lymphoma-a retrospective national multi-center study.

Hodgkin lymphoma (HL), a disease of mostly young patients, also peaks in the elderly. Despite the profound improvement in the outcome of young patients, in the elderly, 5-year progression-free survival (PFS) rates are under 70%. Interim PET-CT (iPET) is known to be highly predictive for PFS in young HL patients, but it has not been sufficiently validated in the elderly patient population. In this multi-center collaboration, all consecutive elderly patients (age ≥ 60) diagnosed with HL between 1998 and...

Fractionated stereotactic body radiotherapy for up to five prostate cancer oligometastases: interim outcomes of a prospective clinical trial.

Stereotactic body radiotherapy (SBRT) can delay escalation to systemic treatment in men with oligometastatic prostate cancer. However, large, prospective studies are still required to evaluate the efficacy of this approach in different patient groups. This is the interim analysis of a prospective, single institution study of men relapsing with up to five synchronous lesions following definitive local treatment for primary prostate cancer. Our aim was to determine the proportion of patients not requiring tre...

Cultural Theory's Contributions to Risk Analysis: A Thematic Review with Directions and Resources for Further Research.

Cultural theory (CT) developed from grid/group analysis, which posits that different patterns of social relations-hierarchist, individualist, egalitarian, and fatalist-produce compatible cultural biases influencing assessment of which hazards pose high or low risk and how to manage them. Introduced to risk analysis (RA) in 1982 by Douglas and Wildavsky's Risk and Culture, this institutional approach to social construction of risk surprised a field hitherto focused on psychological influences on risk percept...

Evaluating the impact of a healthy corner store initiative on food access domains.

Healthy Corner Store Initiatives (HCSIs) are one food access strategy that aims to improve food environments by supporting local food stores in expanding their healthy offerings. This study presents the evaluation results from one such HCSI. Fresh Foods Here evaluated its network of eight stores in Columbus, Ohio between November 2013 and August 2014. Data from invoices, inventories, rapid market assessments, and customers surveys were analyzed for evidence of impact on the service delivery and personal dom...

Opioid addiction, opioid addiction treatment, and HIV infection.

Available data indicate that opioid substitution treatment can successfully reduce rates of HIV transmission and that patients receiving such treatment can adhere to therapies for HIV, hepatitis C, and tuberculosis infection. Integration of opioid substitution treatment into the HIV clinic setting can make such treatment easier and improve retention in treatment. This article summarizes a presentation by R. Douglas Bruce, MD, MA, MS, at the IAS-USA continuing education program held in Chicago, Illinois, in ...

Questionable interim analyses in the Goal-Directed Perfusion Trial study of goal-directed perfusion.

Characteristics of 2017 GOLD COPD group A: a multicenter cross-sectional CAP study in Japan.

The 2017 GOLD ABCD classification shifts patients from groups C-D to A-B. Group A was the most widely distributed group in several studies. It would be useful to understand the characteristics for group A patients, but little has been reported concerning these issues.

Appearance of large scavenger receptor A-positive cells in peripheral blood: A potential risk factor for severe exacerbation of chronic obstructive pulmonary disease.

Fresh peripheral blood (PB) samples from 432 outpatients with stable chronic obstructive pulmonary disease (COPD) were examined. Patients were classified into Group A (large SRA cells were undetected) and Group B (large SRA cells were detected) and followed-up for 1 year. Patients were further subdivided according to Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage. Cox proportional hazard model had shown that Gold, Group, home oxygen therapy (HOT), and treatment were significant predicto...

The apoptotic effects of bisphenol A exposure on the rat ovary: an experimental study.

Bisphenol A (BPA) is a key endocrine-disrupting chemical (EDC) in the manufacturing industry. It is found in the structure of compounds such as polycarbonate and epoxy in combination with other chemicals. Our objective was to investigate the effect of BPA on rat ovaries. A total of 32 female rats were divided into four equal groups: In group 1 (control), vehicle was administered; in group 2, BPA 50 μg/day was administered intraperitoneally; in group 3, BPA 100 mg/kg/day was administered intraperitoneally...

Lamina cribrosa morphology in glaucomatous eyes with hemifield defect in a Korean population.

To compare regional variations in lamina cribrosa (LC) curvature and depth between healthy eyes (Group 1) and naïve eyes with primary open-angle glaucoma (POAG) having superior (Group 2), inferior (Group 3), and both (Group 4) hemifield retinal nerve fiber layer (RNFL) defects.

New stoichiometric compounds of Au-Hg system under high pressure.

We have extensively explored Au-Hg system under high pressure in the range of 0-300 GPa with and without spin-orbit coupling (SOC) inclusion. The variable-composition methodology has been used as employed in Universal Structure Predictor: Evolutionary Xtallograpgy (USPEX) in combination with Vienna Ab Initio Simulation Package (VASP). We have predicted several thermodynamically stable stoichiometric compounds of Au-Hg system, i.e. Au3Hg (space group 194, P63/mmc), AuHg (space group 62, Pnma), AuHg2 (space g...

Returning to home cage serves as an effective reward for maze learning in rats.

This study examined the effectiveness in rats of 'returning to home cage' as a reward for learning a Lashley III maze. Rats could return to their home cage directly (Direct HC group) or they could be removed manually by an experimenter from the maze's goal box (Indirect HC group). In the third group, hungry rats received a food reward in the goal box (Food group). The Direct HC group reliably learned the maze and its performance was very similar to that of the Food group. In contrast, performance by the Ind...

Histopathology re-examination of the NTP toxicity/carcinogenicity studies of tert-butyl alcohol to identify renal tumor and toxicity modes of action.

Tert-butyl alcohol (TBA) targets the rat kidney following repeated exposures, including renal tubule tumors. The mode of action (MOA) of these tumors, concluded by a pathology working group, involves both alpha2u-globulin nephropathy (α2u-gN) and exacerbated chronic progressive nephropathy (CPN), but has been disputed and an undefined MOA proposed. This study further reviews the histology slides of male and female rat kidneys from the NTP drinking water 13-week toxicity and 2-year carcinogenicity studies, ...

Emergent Invasive Group A Streptococcus dysgalactiae subsp. Equisimilis, United States, 2015-2018.

The term group A Streptococcus is considered synonymous for the species Streptococcus pyogenes. We describe an emergent invasive S. dysgalactiae subspecies equisimilis lineage that obtained the group A antigen through a single ancestral recombination event between a group C S. dysgalactiae subsp. equisimilis strain and group A S. pyogenes strain.

Post-MI treatment with G-CSF and EPO-liposome with SLX repairs infarcted myocardium through EPCs mobilization and activation of prosurvival signals in rabbits.

We investigated whether combination therapy of G-CSF and erythropoietin (EPO)-liposome with Siaryl Lewis X (SLX) is more cardioprotective than G-CSF or EPO-liposome with SLX alone. For the purpose of generating myocardial infarction (MI), rabbits underwent 30 minutes of coronary occlusion and 14 days of reperfusion. We administered saline (control group, i.v.,), G-CSF (G group, 10 μg/kg/day × 5 days, i.c., starting at 24 hours after reperfusion), EPO-liposome with SLX (LE group, i.v., 2500 IU/kg E...
