Low-pressure Lumbar Provocation Discography According to Spine Intervention Society/International Association for the Study of Pain Standards Does Not Cause Acceleration of Disc Degeneration in Patients with Symptomatic Low Back Pain: A 7 Year Matched Cohort Study.
MINI: This study investigated if low-pressure provocation discography results in accelerated disc degeneration, disc disruption, and disc herniation at long-term follow-up in patients with low back pain in clinical practice. The results demonstrated disc degeneration, disc disruption, and disc herniation at a similar rate to corresponding discs in matched-control patients.
Different transforaminal endoscopic approaches have been developed for the surgical treatment of lumbar disc herniations: Kambin (intradiscal), Yeung (intraforaminal intra-discal), Ruetten (extreme lateral), TESSYS approach (intraforaminal extradiscal approach) and modifications. The operative technique, for the treatment of foraminal and extraforaminal lumbar disc herniations through these surgical approaches, has not been well described in literature. Moreover, each of these surgical approaches has limita...
In this case report, we present a novel stabilization technique of the pubic symphysis using an internal spinal fixator in a 78-year-old morbidly obese woman having a pelvic disruption type B1.1 (AO classification). We treated the disruption using an internal fixator to reduce the extent of the incision and soft-tissue damage.
The dynamic disc model refers to the ability of a spinal disc's position to be manipulated by body postures and movements. Research on lumbar discs has indicated movement of the anterior and posterior disc that correlates with posture of the spine. The aim of this study was to assess whether, despite its structural differences, the cervical disc responds to flexed and extended postures in a similar fashion to the lumbar disc.
The "Discrimination and Stigma Scale" (DISC) was the first instrument specifically designed to evaluate reported experiences of discrimination by people with mental disorders. This study aims to validate DISC-12 version in Spanish population with Schizophrenia and, as specific objectives, to do the external validation with the Self-Stigma Questionnaire (SSQ) scale and Link PDD scale and to validate their internal consistency, temporal and inter-rater reliability.
Proteoglycan (PG) is a major component of the intervertebral disc extracellular matrix (ECM) that acts to hydrate the disc nucleus. Early detection of PG degradation is valuable for both diagnosis and preclinical research of intervertebral disc degeneration (IVDD).
In this report, we describe an extremely rare case of a giant pseudoaneurysm of the internal iliac artery causing marked destructive changes at a disc space and mimicking discitis-osteomyelitis. Only two other cases of pseudoaneurysms extending into the disc space have been reported, and those arose from the aorta and were relatively straightforward to diagnose. Our case is unique because the pseudoaneurysm arose from the internal iliac artery, an artery that is not usually included in the field of view of ...
Intervertebral disc extrusion is a common disease affecting chondrodystrophic dogs. It has been reported that fenestration of thoracolumbar intervertebral discs reduces recurrence of disc extrusion and is associated with a low complication rate. One complication reported is iatrogenic introduction of disc material into the canal directly following fenestration. This study aimed to ascertain if, and at what frequency, additional disc material may be introduced into the vertebral canal by fenestration of t...
Tissues in the intervertebral disc have a large capacity to absorb water, partially due to the high glycosaminoglycan (GAG) content, which decreases linearly from the nucleus pulposus (NP) in the center to the outer annulus. Our recent work showed that fiber network and GAG distribution contributes to development of residual stresses and strains that were compressive in the inner annulus to tensile in the outer annulus. GAG loss in the inner annulus, as observed with early to moderate degeneration, reduced ...
Brain functional network properties are globally disrupted in multiple musculoskeletal chronic pain conditions. Back pain with lumbar disc herniation is highly prevalent and a major route for progression to chronic back pain. However, brain functional network properties remain unknown in such patients. Here, we examined resting-state fMRI-based functional connectivity networks in chronic back pain patients with clear evidence for lumbar disc herniation (LDH-CP, n = 146), in comparison to healthy controls (H...
Surgery for tethered spinal cord caused by thickened filum terminale (FT) is frequently performed through S1 laminectomy based on the assumption that the internal FT (FTi) fuses with dura mater at S2 vertebral level. Literature on specific study for the site of its fusion and dural sac (DS) termination was rather limited. Moreover, there is no large anatomical study in Asian population. To determine the anatomy, examination of the FTi fusion site, as well as the region at which DS ended, was undertaken. Fro...
The alteration of circadian rhythms in the postoperative period has been demonstrated to influence the outcomes. With this narrative review we would revise how anesthesia, surgery and intensive care can interfere with the circadian clock, how this could impact on the postsurgical period and how to limit the disruption of the internal clock.
Long-Term Comparison of Health Care Utilization and Reoperation Rates in Patients Undergoing Cervical Disc Arthroplasty (CDA) and Anterior Cervical Discectomy and Fusion (ACDF) for Cervical Degenerative Disc Disease.
Retrospective cohort analysis BACKGROUND: Anterior cervical discectomy and fusion (ACDF) has been the gold standard procedure for single level degenerative disc disease (DDD). Recently, cervical disc arthroplasty (CDA) has become increasingly prevalent as an alternate intervention.
Myelinated nerve fibers (MNF) are characterized as whitish, relatively sharply demarcated, feather-like structures located in the retinal nerve fiber layer. MNF are located quite frequently in contiguity with the optic nerve head. This may lead to a diagnostic dilemma by preventing clear visualization of the optic disc margins. We herein describe the utility of multicolor imaging (MCI) in identifying optic disc anatomy in a case of MNF contiguous to the disc. MCI and infrared reflectance were superior to co...
Abnormal cervical curvature and cervical disc herniation are closely related to neck pain and should be taken into account before any treatment. However, studies have rarely reported on the correlation between cervical lordosis and cervical disc herniation in patients with neck pain. Therefore, in this study, we collect young neck pain patients with abnormal cervical curvature to evaluate the relationship between cervical lordosis and cervical disc herniation.Three hundred patients below 40 years old with n...
Cervical disc arthroplasty (CDA) is now a widely accepted alternative to anterior cervical interbody fusion, which is known to reduce normal cervical motion and increase the incidence of adjacent segment disease. Although multiple studies report the use of cervical disc arthroplasty to treat multi-level cervical disease, this is the first report in the literature detailing the placement of multiple, non-contiguous artificial discs.
We aimed to investigate mechano-sensitivity at the afferent nerve fibers projecting to degenerated intervertebral disc (IVD) and nociceptive behaviour in a rat model of low back pain (LBP).
Injections are associated with a certain amount of pain, the tolerance of which can vary between individuals. With regard to noninvasive pain control techniques in subcutaneous injections, few studies with adequate levels of evidence and design quality exist to support any specific analgesic method.
Quantitative magnetic resonance imaging was used to determine partition coefficients and characteristic time constants for diffusion of MRI contrast agents in disc tissue.
Heavy metals, such as lead can cause optic neuropathy. Optic disc neuropathy due to lead intoxication has previously been reported. We report a rare case of lead toxicity-induced optic neuropathy presenting with bilateral hemorrhagic optic disc swelling.
Lumbar 3-joint complex degeneration is a multifactorial, pathological process. Previous studies included insufficient quantitative analyses to prove the relationship between disc degeneration and facet joint osteoarthritis (OA). We assessed the correlation between intervertebral disc and lumbar facet joint degeneration using computed tomography (CT) and magnetic resonance imaging (MRI) parameters.A total of 152 participants who underwent conventional MRI and CT in the clinostat position were included in thi...
Inflammation has been associated with a number of pathological conditions including intervertebral disc (IVD) degeneration, increased risks of low back pain and other spinal diseases. Downregulating disc inflammation may be a strategy to reduce degeneration and more importantly back pain. Interleukin (IL)-4 was first discovered as a T-cell secreted factor that enhanced the proliferation of anti-IgM stimulated B cells and is now known as a cytokine that can stimulate cell proliferation and differentiation, t...
